Eligibility

All ages, with proof of:

• Before-tax household income under 200% of the
• Federal Poverty Level, ($20,420 for one person,
• $27,380 for two people, etc.)
• Residency within the clinic service area
• Assets under $2,000 ($20,000 if in a retirement fund)
